Here is the Annual List of Violations that you are asked to fill out each year.  This form asks that you list all moving violations for the prior three years.

This document will be one of several which the safety department will use to perform an annual review of your driving performance, to determine – again – that you are qualified to drive a commercial vehicle in interstate commerce today as you were the day you first contracted to Mamo Transportation.
